Have a primary residence in a county where Blue Cross Medicare Advantage Dual Care Plus (HMO SNP) is offered. Eligible counties are Bernalillo, Chaves, Curry, Dona Ana, Otero, Quay, Roosevelt, Sandoval, Santa Fe, Taos, Torrance, and Valencia.
Depending on your level of Medicaid eligibility, you may not have any cost-sharing responsibility.
